Out-of-Network
If you have another insurance plan listed and have out of network benefits, we may be able to submit the claim directly to your insurance plan for you. You will be responsible to pay the self-pay fees at the time of visit. For more information on fees please contact us to discuss.
We are currently unable to accept Blue Cross Blue Shield plans.

Cancellation Policy
Appointments cancelled less than 24 hours in advance are subject to a cancellation fee of up to $100. After two (2) cancellations, the late cancellation fee will be the full visit fee.
To cancel an appointment less than 24 hours in advance, please contact our office. Cancellations within this 24-hour window are disabled on our Online Scheduler.
