These Terms and Conditions of Use ("Terms") govern your access to and use of the website operated by Mark Hall Nurse Practitioner in Psychiatry PLLC ("we," "us," or "our"), also doing business as Mark Hall NP in Psychiatry PLLC, including all content, features, and services available at markhallnp.com (the "Site"). By accessing or using the Site, you agree to be bound by these Terms. If you do not agree, please do not use the Site.

We reserve the right to modify these Terms at any time. Changes are effective upon posting. Continued use of the Site following any changes constitutes your acceptance of the revised Terms. It is your responsibility to review these Terms periodically.

Nothing on this Site creates a provider-patient relationship between you and Mark Hall NP in Psychiatry PLLC or any of its clinicians. A provider-patient relationship is only established through a formal intake process and your provider's explicit acceptance of you as a patient.

Mark Hall NP in Psychiatry PLLC is a psychiatric nurse practitioner practice located in New York. Services described on this Site are available only to patients who are physically located in New York State at the time services are rendered, as required by applicable New York State licensing and telehealth regulations.

Descriptions of services on this Site are for informational purposes only and do not guarantee availability, insurance coverage, or clinical appropriateness for any individual. Contact our office to discuss whether our services are right for your needs.

The insurance information on this Site is provided as a general guide only. You acknowledge and agree that:

• Insurance plan participation is subject to change at any time without advance notice on this Site; contact our office or your insurer to confirm current participation status before scheduling
• Acceptance of an insurance carrier does not mean all plans, products, or benefit tiers offered by that carrier are accepted; coverage varies by plan type, employer group, and individual policy
• Even if we participate with your insurance plan, your actual benefits, cost-sharing obligations (copayments, deductibles, coinsurance), and coverage determinations are made solely by your insurance company, not by us
• It is your responsibility to verify your benefits, eligibility, and network status with your insurance company prior to your appointment by calling the member services number on the back of your insurance card
• We are currently unable to accept Blue Cross Blue Shield plans; patients with BCBS coverage should contact us to discuss out-of-network options
• If you have out-of-network benefits, we may be able to submit claims directly to your insurance on your behalf; however, you will be responsible for paying self-pay fees at the time of your visit
• Insurance coverage for telehealth services may differ from in-person coverage; verify telehealth benefits with your insurer prior to a virtual appointment

Self-Pay Patients. We offer self-pay fees for patients without insurance or whose insurance we do not accept. For current self-pay fee information, please visit our Insurance & Fees page or contact our office. Self-pay fees are subject to change.

Our cancellation and no-show policy differs by appointment type. Please review the policy that applies to your visit.

5.1   Medication Management Appointments

Cancellations must be made at least 24 hours in advance. Appointments cancelled with less than 24 hours' notice, or missed without prior notice, are subject to a cancellation or no-show fee of up to $100.

5.2   Psychotherapy Appointments

Cancellations must be made at least 48 hours in advance. Psychotherapy appointments cancelled with less than 48 hours' notice, or missed without prior notice, will be billed at the full visit fee. This fee is not billable to insurance and is the patient's sole financial responsibility.

5.3   Late Arrivals

If you arrive more than 15 minutes late to an in-person or virtual appointment without prior notice, the appointment may be treated as a no-show at the provider's discretion and the applicable fee above may apply.

5.4   General

Cancellation and no-show fees are not covered by insurance and are the patient's sole financial responsibility. Repeated no-shows or late cancellations may affect your ability to schedule future appointments. For questions, call (646) 500-8627.

All content on this Site, including but not limited to text, graphics, logos, images, and the compilation thereof, is the property of Mark Hall Nurse Practitioner in Psychiatry PLLC or its content suppliers and is protected by applicable United States and international copyright and intellectual property laws.

You may access and view Site content for your personal, non-commercial use only. You may not:

• Reproduce, distribute, modify, or create derivative works from Site content without our prior written consent
• Use Site content for any commercial purpose
• Remove or alter any copyright, trademark, or other proprietary notices
• Frame or mirror any portion of the Site without our express written consent

Submitting a contact or inquiry form through this Site does not create a provider-patient relationship, does not constitute a request for an appointment, and does not guarantee a response. For appointment requests, please use the Patient Portal scheduling tool or call our office directly.

In no event shall our total liability to you for all claims arising from your use of the Site exceed $250. Some jurisdictions do not allow the exclusion of certain warranties or limitation of liability; in such jurisdictions, our liability shall be limited to the fullest extent permitted by law.

If any provision of these Terms is found to be unenforceable by a court of competent jurisdiction, the remaining provisions shall continue in full force and effect.
